The Department of Health Care Policy and Financing has awarded the Regional Care Collaborative Organization (RCCO) contract for regions 2, 3, and 5 to Colorado Access.

The contract arrangement is a Primary Care Case Management (PCCM) model. The contracting period is slated to be from February 1, 2011 through June 30, 2012 with the potential to expand services throughout the region over the subsequent year(s). Expansion is dependent upon the successful attainment of Department declared outcomes measures.

As the Department’s contracted vendor, Colorado Access will be working closely with the physicians and community service organizations to provide coordinated case management and administrative services to manage the State’s Regional Care Collaborative Organization members in Adams, Arapahoe, Douglas, Denver, Weld, Morgan, Logan, Sedgwick, Phillips, Washington, Yuma, Lincoln, Kit Carson and Cheyenne counties.

Key activities include:

  • Provision of a primary care physician network (Medical homes for all RCCO members)
  • Provision of primary care case management - Enhanced Primary Care Case Management means a clinical program comprised of the location, coordination and monitoring of primary health services including care coordination, case management activities, health education, consumer advocacy and empowerment, and health promotion
  • Outcomes and performance management
  • Medical management
  • Communications management
  • Provider relations and network management
  • Eligibility and enrollment management
